Consent processing of personal data

I hereby and within the framework of the provisions of Organic Law 3/2018, of 5 December, on the Protection of Personal Data and guarantee of digital rights and Regulation (EU) 2016/679 of the European Parliament and of the Council, of 27 April 2016;  

I declare that I have contacted IVF Health Holding, SLU (IHH), a Spanish company with CIF B54434733 with legal address at Avda. Ansaldo, 12, 03540, Alicante, in order to receive free medical counselling on assisted human reproduction treatments and to be referred, where appropriate, to the IVF center that they may recommend me, arrange as many consultations I might need and coordinate and monitor the treatment in close collaboration with the selected IVF center.  

I authorise IHH to process the data necessary for the purpose of guidance and advice on medical reproductive treatments, and, where appropriate, monitoring and accompaniment in the assisted reproduction treatment that may be carried out in the clinic that may be selected. Therefore, I hereby authorise IHH to transfer my data to this clinic in order to organise a first medical visit.  

I also authorise IHH to have access to all the data and/or documents that I provide to the clinic that may be selected during the medical treatment and/or that it may generate in the framework of the same (reports, tests, analyses, sample maintenance, images and time-lapse…etc.) for which I require the Clinic to provide these data and documents as soon as possible in order to better manage and follow up my medical treatment, for the follow-up and accompaniment.  

I also AUTHORIZE IHH to process my personal data in order to communicate to me advantages, clinical trials, promotions or news that may be applicable to me.  

1. Purpose of data processing: Personal data will be used for the purpose of providing counselling and medical advice and monitoring the treatment I may undergo at the Clinic. Likewise, if I expressly authorize to be kept informed about promotions, clinical trials, offers through the channels provided. 
2. 2. Conservation period: The data will be kept for the established legal periods in strict confidentiality. The contact details will be kept as long as you do not request your right to erase, oppose, or unsubscribe. 
3. Legitimation: Legitimation is the fulfillment of legal obligations and the consent granted. 
4. Recipients: My data may be transferred to the clinic that may be selected. My data will not be transferred to any other company, entity o r natural or legal person, unless such transfer is required by law. 
5. International transfers: Not applicable. 
6. Confidentiality: Personal data that will be treated confidentially. IHH staff and collaborators have previously committed themselves to the duty of confidentiality. Technical and organizational measures have been put in place to limit access to authorized persons only. 
7. Rights: I have been informed that I can exercise the rights of access, rectification, deletion, limitation, portability and opposition to your personal data, requesting it in writing to the DPO of IHH through the email agarcia@legitec.com, having at my disposal models for the exercise of the same. If I consider my rights violated, I have been informed that I can claim in www.aepd.es.
